Welcome to Annville-Cleona Sports Medicine
Our athletic training and sports physician services are provided to our school district by the Penn State Hershey Medical Center.
Our Certified Athletic Trainer is Tanya Deihl, who is a graduate of California University of Pennsylvania and Penn State University. If you have any questions or concerns related to the medical care of your athlete/child please call 717-867-7710 or email at Tdeihl@acschools.org. If the Athletic Trainer is not available, please leave a message and your call will be returned in a timely manner.
Important Information Regarding Athletic Physicals
*All student athletes are required to have a new sports physical for each academic year.
*Athletic physicals for the upcoming year may not be performed until after June 1st.
*After an athlete obtains a physical for one season, only a re-certification form needs to be completed for each subsequent sport season. The re-certification form does not require a physicians signature unless the athlete had an injury that was seen by a physician during the previous sport season.
*All physical information is completed on Family ID. If you do not have a Family ID account, please visit www.familyid.com to create an account.
