After an athlete obtains a physical for one season, only a re-certification form needs to be completed for each subsequent sport season. When the athlete suffers an injury, the recertification form must be signed by a physician.

Any student athlete who transfers to Annville-Cleona from another school, must print this form, complete the student section, acquire a signature from his/her previous school and return the completed form to the athletic office, prior to participating in any A-C athletic program.

If you are a Home/Cyber School student who plans to participate in the Annville-Cleona athletic program, you must complete this form and turn it in to your coach prior to the start of your season.
