Applicants must fill out separate Act 168 Sexual Misconduct/Abuse Disclosure Release forms for each former employer where he/she had direct contact with children, regardless of whether the employer was a school entity. A separate Act 168 form must also be filled out for any current employer(s) regardless of whether the applicant has direct contact with children. The forms must be submitted to the Business Office after completing the employer information section and section one of the form. Annville-Cleona School District will send the form to the current/former employers for the completion of section two.
